    Toll-like Receptors Activity on Circulating Monocytes and Neutrophils in Patients With Obstructive Sleep Apnea Hypopnea Syndrome.

    Session TitleComorbidities of OSA PostersSESSION TYPE: Poster PresentationsPRESENTED ON: Saturday, March 22, 2014 at 01:15 PM - 02:15 PMPURPOSE: To provide new insight into the expression of TLR2 and TLR4 from OSAHS.MethodsSubjects from the pations,who were suspected OSASH,were recruited into the cohort study.Surface expressions of TLR2 and TLR4 on circulating monocytes and neutrophils were measured by flow cytometry.ResultsSubjects(n=48) had divided into OSAHS group(n=37),simple snoring group(n=6) and positive airway pressure treatment group(n=5).A level of TLR2 on neutrophils was up-regulated in OSAHS group,while TLR4 was down-regulated.Linear regression analysis revealed time spent with SpO2<90% was associated with the expression levels of TLR2 and TLR4 on monocytes before sleep(r=0.309, 0.302; P=0.044, 0.049). The positive correlation was shown between arousal index and the expression levels of TLR4 on monocytes before and after sleep, as well as the levels of TLR4 on neutrophils after sleep, with significant statistic (r=0.421, 0.392, 0.350; P=0.005, 0.010, 0.023).Statins was showed to influence the expression levels of TLR4 on monocytes after sleep. The up-regulation was observed in statins group, being compared with the down-regulation in non-statins group. As a result, significant statistic difference was showed between this two group in expression levels after sleep and D-value through the sleep (after sleep: non-statins group vs statins group, 29.89%±15.55 vs 39.16%±19.46, P=0.001; D-value through the sleep: non-statins group vs statins group, 4.37%±9.87 vs -7.95%±9.58, P=0.000.ConclusionsTLR2 and TLR4 on monocytes might participate in chronic inflammatory in OSAHS, while TLR4 on monocytes might also play a role in acute inflammatory in OSAHS.The hypoxia load (esp. the time load of hypoxia) and arousal would have a closer relationship with TLR2 and TLR4 in OSAHS.Statins might down-regulate the expression levels of TLR4 on monocytes after sleep in OSAHS for its anti-inflammation effect, which might not be observed without steady state plasma concentration.Clinical ImplicationsTLR2 and TLR4 on neutrophils might participate in acute inflammatory process in OSAHS.DisclosureThe following authors have nothing to disclose: Junjun HuangNo Product/Research Disclosure Information.
