    Higher AHI Predicts More Exacerbations in Indian Patients With Bronchial Asthma With Overlap of Obstructive Sleep Apnea and Not Affected by PAP Therapy.

This study correlates number of exacerbations with AHI in 24 stable asthmatics with overlap of OSAMETHODS: 24 stable patients' of BA with overlap of OSA diagnosed on PSG, using or not using CPAP, attending outpatient clinic of Metro Centre for Respiratory diseases were retrospectively analysed for number of exacerbations in last one year using SPSS software.Results18 out of 24 patients were not using CPAP(75%). The average pre-bronchodilator FEV1, EPSS, AHI and number of exacerbations in the previous year in those using CPAP was 1.21L(SD=0.49), 5.83(SD=1.72), 19.93(SD=10.37), 1.66(SD=1.21) respectively; whereas for those not using CPAP therapy were 1.35L(SD=0.54), 8.61(SD=5.08), 22.40(SD=14.41) and 2.38(SD=0.91) respectively. Strong positive correlation was observed between number of exacerbations and AHI irrespective of being on treatment or not(r=0.845;P=0.034 in those using PAP therapy and r=0.687;P=0.002 in those not using PAP therapy).ConclusionsIncreased number of exacerbations were observed in patients with high AHI in Overlap syndrome, not affected by therapy.Clinical ImplicationsSeveral western studies have shown an improvement in asthma symptoms after the initiation of CPAP. Our study showed a direct relationship between BA exacerbations and AHI, but has small number of patients in group using CPAP.
