    AbstractA 15-year-old adolescent boy was referred by an orthopaedic surgeon for treatment of right gluteal and thigh pain. Following treatment that reduced his pain, he experienced acute onset of bilateral radiculopathy. He was referred back to the physician, who ordered radiographs and magnetic resonance imaging, which identified a grade IV isthmic spondylolisthesis at the L5-S1 level.
