Comparative StudyComparison of EUS-guided rendezvous and precut papillotomy techniques for biliary access (with videos).
Precut papillotomy after failed bile duct cannulation is associated with an increased risk of pancreatitis. EUS-guided rendezvous drainage is a novel alternative technique, but there are no data comparing this approach with precut papillotomy. ⋯ In this study, the EUS-guided rendezvous technique was found to be superior to precut papillotomy for single-session biliary access. Prospective randomized trials are needed to confirm these preliminary but promising findings.

Comparative Study Controlled Clinical TrialBlinded nonrandomized comparative study of gastric examination with a magnetically guided capsule endoscope and standard videoendoscope.
Passive video capsule endoscopy is the criterion standard for small-bowel exploration but cannot be used for the large gastric cavity. We report the first blinded comparative clinical trial in humans comparing a magnetically guided capsule endoscope (MGCE) and a conventional high-definition gastroscope. ⋯ Diagnostic results were similar for the 2 methods. After some technical difficulties related to gastric expansion or presence of mucus had been overcome, this study opened a new field for noninvasive gastric examination in countries where high gastric cancer incidence demands a screening tool.

Randomized Controlled TrialStrategies for training in diagnostic upper endoscopy: a prospective, randomized trial.
Training simulators have been used for decades with success; however, a standardized educational strategy for diagnostic EGD is still lacking. ⋯ Structured simulator training supplementing clinical training in upper endoscopy appears to be superior to clinical training alone. Simulator training alone does not seem to be sufficient to improve endoscopic skills.
