High-grade malignant mixed adenoneuroendocrine carcinoma (MANEC) is a highly malignant combined neoplasm formed by an adenocarcinomatous component and a poorly differentiated (Grade 3) neuroendocrine (NE) carcinoma. ⋯ The predominant tumor component in primary tumors was a prognostic factor and could predict tumor emboli and liver metastases pathology in high-grade malignant MANECs.

In implant therapy, the adequate state of peri-implant tissue health and soft-tissue aesthetics is the essential criterion of restorative success. The need for keratinized mucosa for the maintenance of peri-implant health and soft-tissue integration remains a debated issue. The aim of this paper is to provide a narrative review of the current literature concerning the significance of keratinized mucosa with respect to the clinical parameters of monitoring oral hygiene practice and tissue status. ⋯ The evidence related to the effect of keratinized mucosa on the changes of attachment or bone levels is limited, and conclusions could not be drawn at present. Further, this review found that a band of keratinized mucosa was not absolutely necessary for the maintenance of peri-implant tissue, whereas lack of adequate keratinized mucosa around the implant might impede proper oral hygiene performance and compromise the aesthetic results. In conclusion, because there is a wide variety of clinical features in patients pursuing implant therapy, individual consideration of treatment strategies for the patient with minimal keratinized mucosa is recommended.

This study investigated whether the time from emergency room registration to appendectomy (ETA) would affect the incidence of perforation and postoperative complications in patients with acute appendicitis. ⋯ In our study, it was observed that although longer ETA was associated with longer hospitalization, ETA was not correlated with postoperative complications. Our results inclined toward the position that appendectomy can be performed as a semielective surgery.

To increase the chance of restoring spontaneous circulation, cardiopulmonary resuscitation (CPR) with high-quality chest compressions is needed. We hypothesized that, in a municipal hospital emergency department, the outcome in nontraumatic out-of-hospital cardiac arrest patients treated with standard CPR followed by mechanical chest compression (MeCC) was not inferior to that followed by manual chest compression (MaCC). The purposes of the study were to test our hypothesis and investigate whether the use of MeCC decreased human power demands for CPR. ⋯ No difference was found in early survival between standard CPR performed with MeCC and that performed with MaCC. However, the use of the MeCC device appears to promote staff availability without waiving patient care in the human power-demanding emergency departments of Taiwan hospitals.

Changes in endothelial progenitor cell subsets in normal pregnancy compared with preeclampsia.
The results of studies measuring the number of endothelial progenitor cells (EPCs) in normal pregnancies and in preeclampsia have been highly controversial or even contradictory because of cross-sectional designs and different methodologies enumerating three distinct subsets of EPCs: circulating angiogenic cells (CAC), colony-forming unit endothelial cell (CFU-ECs), and endothelial colony forming cells (ECFCs). To provide a clear explanation for these underlying controversies, we designed a prospective study to compare the number of all EPC subsets between three trimesters of normal gestation and a case-control study to compare these values as preeclampsia occurs with those from gestational age (GA) matched normal pregnancy. ⋯ Although we did demonstrate an increase in absolute counts of CACs and ECFCs in preeclampsia, fewer colony formation capacities indicated a loss in their functional capabilities. By contrast, the number of CACs increased without alterations in colony formation ability in normal pregnancy with the growth of the fetus. Here, by comparing different methodologies to calculate the number of EPC subsets, we could imitate the existing controversy in the literature for such calculations, which may help to elucidate clearer explanations.
