Review Case ReportsWell leg compartment syndrome postorthognathic surgery under hypotensive anesthesia and reverse Trendelenburg position: a case report and literature review.
Well leg compartment syndrome is a rare postoperative condition that occurs as a result of non-traumatic ischemic insult. It has similar pathophysiology to compartment syndrome; however, no definitive etiology is identified. Several risk factors such as patient position, leg position, young age, and high BMI for the development of this condition are considered. ⋯ Increased Awareness of this rare occurrence and predisposition aids in understanding and prompt management. This article provides a rare case that has occurred in reverse Trendelenburg position with less obvious risk factors than typically reported in the literature.

Safety of resection margins in CAD/CAM-guided primarily reconstructed oral squamous cell carcinoma-a retrospective case series.
After resection of malignancies of the jaws, CAD/CAM procedures have become standard for primary bony reconstruction. Even so, these techniques may limit surgical resection safety. Therefore, the aim of the study was to examine osseous as well as soft tissue resection margins after CAD/CAM-guided tumor resections and reconstructions. ⋯ CAD/CAM procedure allows safe tumor resection with the profit of a guided and accurate reconstruction. In contrast to positive soft tissue margins, positive bony resection margins did not increase recurrence parameters.

Observational StudyPre-arthroplastic mandibular distraction osteogenesis for the correction of OSA in TMJ ankylosis: a prospective observational study of 25 cases.
In severe TMJ ankylosis cases, the lack of growth of the mandible creates an anatomically narrow airway with a reduced pharyngeal airway space [PAS] which predisposes these patients towards obstructive apnoea [OSA]. There is evidence in the literature that such patients experience severe discomfort during physiotherapy if such airway abnormalities are not corrected prior to ankylosis release. This eventually leads to non-compliance towards physiotherapy and increases the risk of re-ankylosis. ⋯ Pre-arthroplastic mandibular DO has proved to be a successful modality for treatment of OSA in TMJ ankylosis patients with stable results at 12 months. By resolving the narrow airway and OSA, compliance towards physiotherapy was improved thus reducing the risk of re-ankylosis in the long term.

Randomized Controlled Trial Comparative StudyOral dexamethasone decreases postoperative pain, swelling, and trismus more than diclofenac following third molar removal: a randomized controlled clinical trial.
The aim of this study was to compare the anti-inflammatory potential of two pharmacotherapy protocols based on the parameters of pain, trismus, and swelling, after extraction of third molars. ⋯ Medical protocol with the use of dexamethasone in the postoperative period was more effective in controlling pain, trismus, and swelling, after the extraction of third molars, when compared to diclofenac sodium.

Comparative StudyUtility of intraoperative nerve monitoring in thyroid surgery: 20-year experience with 1418 cases.
The efficacy of intraoperative nerve monitoring is controversial in the literature. This study of a single surgeon's experience seeks to determine if the use of intraoperative nerve monitoring influences recurrent laryngeal nerve injury during thyroid surgery. ⋯ Intraoperative monitoring during thyroidectomy may not prevent injury to the recurrent laryngeal nerve.
