ReviewBuprenorphine therapy: an increasing challenge in oral and maxillofacial surgery.
Suboxone is a 4:1 mixture of buprenorphine and naloxone and Subutex is buprenorphine alone. The high affinity μ-receptor binding of buprenorphine (Suboxone and Subutex) renders other opioids ineffective. Inadequate procedural sedation, inadequate analgesia, and significant drug interactions complicate the treatment of patients taking Suboxone or Subutex. Careful planning and an understanding of buprenorphine pharmacology can minimize potential perioperative complications in patients taking Suboxone or Subutex.

Considerations for planning and designing meta-analysis in oral medicine.
Evidence to address a scientific question is generated through the design of research aiming to answer it. Probably the most often sought design in epidemiologic research is a randomized clinical trial that offers the advantage of controlling for confounders, which may influence the trial outcome. In contrast, observational study designs test hypotheses when clinical trials are difficult to implement. ⋯ Thus, the clinician faces the challenge of making sense of multiple studies that produce results of variable strength when attempting to assess the evidence supporting a treatment. Meta-analytic methods represent an alternative to assess the evidence by pooling the results from multiple studies to increase statistical power. This manuscript describes considerations for planning and implementing meta-analysis in oral medicine.

Randomized Controlled Trial Comparative StudyArticaine (4%) with epinephrine (1:100,000 or 1:200,000) in intraosseous injections in symptomatic irreversible pulpitis of mandibular molars: anesthetic efficacy and cardiovascular effects.
The aim of this study was to compare the cardiovascular effects and the anesthetic efficacy of intraosseous injections of 4% articaine with 1:100,000 epinephrine (EPI100) or 4% articaine with 1:200,000 epinephrine (EPI200). ⋯ The epinephrine concentration did not affect the efficacy of 4% articaine, and both solutions produced a high success level of pulpal anesthesia. Intraosseous delivery by slow speed of injection did not induce significant clinical changes in cardiovascular parameters.

Case ReportsDentomaxillofacial manifestations of mucopolysaccharidosis VI: clinical and imaging findings from two cases, with an emphasis on the temporomandibular joint.
Using a clinical survey, panoramic, cone-beam computed tomography (CBCT), and magnetic resonance (MR) imaging, this study was conducted to ascertain primary maxillofacial abnormalities in patients with mucopolysaccharidosis VI (MPS VI). ⋯ CBCT and MR imaging were needed to observe features that were not clear in conventional radiographs. Both patients reported symptoms in the TMJ and demonstrated involvement during their examinations. A multicenter study is necessary to better document maxillofacial involvement in MPS VI.
