Diagnosis of extrauterine pregnancy with transvaginal ultrasound.
We reviewed all cases of suspected ectopic pregnancy (EP) that had transvaginal ultrasound (TVU) prior to surgery from 1987 to 1989. Of 265 patients who had culdocentesis and laparoscopy for suspected EP, 206 had EP. ⋯ An ectopic embryo was seen at TVU in 28 cases where laparoscopy confirmed the TVU diagnosis. In 163 cases where EP was considered probable at TVU this was confirmed at laparoscopy. 15 cases of EP were overlooked at TVU.

Comparative StudyMode of delivery and lymphocyte beta 2-adrenoceptor density in parturients and newborns.
Twenty-six women with uncomplicated pregnancy at term were selected for the study. Nine had spontaneous vaginal delivery without medication, 8 elective caesarean section under epidural and 9 under general anaesthesia. ⋯ Vaginally delivered newborns have lower lymphocyte beta 2-adrenoceptor density than those delivered by caesarean section. A plausible explanation is the down-regulation of the beta 2-adrenoceptors during labour and delivery.

Case ReportsAnaphylactoid reaction to oxytocin during cesarean section.
A case of an allergic reaction to synthetic oxytocin administered during cesarean section is reported. Epinephrine was more effective in improving the severe hypotension than ephedrine. The incidence of anaphylactoid reaction to oxytocin is very low, but this potential problem must always be kept in mind in daily obstetric practice.

Comparative StudyPostnatal changes in colloid osmotic pressure in premature infants: in healthy infants, in infants with respiratory distress syndrome, and in infants born to mothers with premature rupture of membranes.
Colloid osmotic pressure (COP) of blood plasma during the first 4 days of life was measured in 63 neonates: 16 healthy preterm infants, 36 infants with respiratory distress syndrome (RDS), and 11 infants born to mothers with premature rupture of membranes. The relation between COP and total protein content of blood was significant in all groups over times from 1-3 h to 96 h. COP rose significantly by the age of 3 h compared to COP of umbilical cord plasma in all groups investigated. ⋯ In infants with RDS there was a negative correlation between changes in COP and body weight not seen in the other groups investigated. COP in neonates seems to reflect the compartmentation between vascular and interstitial spaces. Measurement of COP could be clinically useful in assessing hemodynamic adaptation after birth and also in assessing edema formation and water balance in infants with RDS.

Haemodynamic effects of fenoterol for labour inhibition during spinal or epidural anaesthesia.
Haemodynamic investigations, partly using invasive monitoring, concerning interactions between the beta-mimetic effect of fenoterol and the alpha-sympathetic blockade of spinal or epidural anaesthesia were made in a total of 49 patients during pregnancy, during caesarean section and in non-pregnant women. Haemodynamic effects of fenoterol during regional anaesthesia were compared with those during general anaesthesia (n = 17). Epidural or spinal anaesthesia enhance vascular beta-mimetic reactions at the start of tocolysis, leading to a profound but short-lasting decrease in blood pressure. This initial hypotension is more pronounced after regional than after general anaesthesia.
