This study evaluated the use of a telephone triage system in an academic primary care clinic and its impact on communication, patient management and satisfaction. ⋯ In a primary care practice, the telephone can be a major source of communication for practitioners, office staff and patients. The creation of a "telephone clinic" which utilizes nurses and house staff physicians trained and dedicated to telephone communication directly with patients resulted in more efficient management and greater satisfaction for patients.

Interrater reliability for the Four Habits Coding Scheme as part of a randomized controlled trial.
To describe the process for developing interrater reliability (IRR) for the Four Habits Coding Scheme (4HCS) for a heterogeneous material as part of a randomized controlled trial. ⋯ 4HCS may be used as an outcome measure for clinical communication skills in randomized controlled trials.

To provide communication skills education to veterinary professionals in the practice setting and evaluate the training by measuring veterinarian communication pre- and post-intervention. ⋯ Practice-based communication training is novel to veterinary practice. As a case study, generalization of the findings are limited, however the findings support the efficacy of the communication intervention and enhanced utilization of veterinarian-client communication skills by these veterinarians.

This is the narratives of a family physician and a teacher of Medical Ethics, who herself suffers from Gaucher's disease. She was asked by a colleague to give an advice to a couple trying to decide whether to terminate a pregnancy of a fetus "like her". The ethical, emotional and communicational challenges of the conversation are described in this essay.

To deepen the knowledge of frail elderly patients' preferences for participation in medical decision making during hospitalization. ⋯ More time should be given to thorough information and as few people as possible should be involved in the care of frail elderly. Linguistic problems should be identified to make it possible to take the necessary precautions to prevent negative impact on patient participation.
