To estimate the global health expenditure on diabetes among people aged 20-79 years for the years 2010 and 2030. ⋯ Diabetes imposes an increasing economic burden on national health care systems worldwide. More prevention efforts are needed to reduce this burden. Meanwhile, the very low expenditures per capita in poor countries indicate that more resources are required to provide basic diabetes care in such settings.

Meta AnalysisAspirin for primary prevention of cardiovascular events in patients with diabetes: A meta-analysis.
To systematically review trials concerning the benefit and risk of aspirin therapy for primary prevention of cardiovascular events in patients with diabetes mellitus. ⋯ In patients with diabetes, aspirin therapy did not significantly reduce the risk of cardiovascular events without an increased risk of major bleeding, and showed sex-specific effects on MI and stroke.

Comparative StudyEvaluation and comparison of guidelines for the management of people with type 2 diabetes from eight European countries.
The most recent nationally recognised guidelines for type 2 diabetes from eight European countries (Belgium, England/Wales, France, Germany, Ireland, Italy, the Netherlands and Sweden) were compared. The Appraisal of Guidelines for Research and Evaluation (AGREE) instrument was used for quality assessment. Details of recommendations for key process and outcome indicators were also extracted. Appraisal and data extraction were conducted independently by two researchers. ⋯ Our findings suggest that there are some areas of good practice relating to guideline development where more attention is needed. Despite a substantial degree of consensus for specified targets, observed differences at detailed level suggest a lack of consistency in relation to some aspects of the information provided to clinicians across Europe.

Bedside capillary blood glucose measurements in critically ill patients: influence of catecholamine therapy.
To evaluate the effects of catecholamine therapy on the accuracy of capillary glucose measurements in hyperglycemic patients. ⋯ In critically ill patients treated with catecholamine capillary fingerstick blood glucose measurement seems inaccurate. Earlobe sampling does not improve accuracy.

Case ReportsDoes glucose infusion exacerbate metformin-associated lactate acidosis? A case report.
Metformin-associated lactate acidosis is a rare, but life-threatening complication of a widely prescribed medication. We describe here a case of metformin-associated lactate acidosis that rapidly deteriorated probably as a consequence of concomitant glucose infusion that was initiated to correct sulphonylurea-induced hypoglycemia.
