Political and economic unfairness in health system of Pakistan: a hope with the recent reforms.
For the last few years, Pakistan's health system has faced numerous challenges pertaining to human resource and its deployment, resource allocation among the different tiers of the health care system, infrastructure development and unfair access to care. The enactment of the recent constitutional amendment has made the health system's situation even more uncertain than before. ⋯ In the post-devolution scenario, provinces muLst think systematically how to deal with the capacity issues to manage different components of health care system. Nonetheless, as a country, collective actions would be required to avoid any pitfalls, while approaching Millennium Developmenit Goals by 2015.

Presentation of maxillofacial injuries in the 2005 earthquake victims.
Earthquakes cause a lot of damage to life and property. Maxillofacial injuries constitute an important proportion of injuries in earthquakes and some 13% of the trauma patients after the earthquakes suffer from maxillofacial injuries. The objective of this study was to assess the presentation of maxillofacial injuries in earthquake victims. ⋯ Although considerable number of patients with fracture of multiple facial bones were present in the earthquake victims but isolated facial bone fractures and soft tissue injuries were the commonest type of injuries.

Randomized Controlled TrialOral clonidine for attenuation of haemodynamic response to laryngoscopy and endotracheal intubation in known hypertensive patients.
Sympathetic response associated with laryngoscopy and endotracheal intubation is recognised as a potential cause for a number of complications especially in hypertensive patients. Various methods have been used to attenuate these haemodynamic responses; however most of the studies are in normotensive patients. The aim of our study was to compare the effect of oral clonidine and 1/V fentanyl with oral placebo and I/V fentanyl in attenuating the haemodynamic responses to laryngoscopy and intubation in known hypertensive patients. ⋯ Clonidine, under the present study design attenuates the haemodynamic response to laryngoscopy and endotracheal intubation in known hypertensive patients.

Prevalence, evaluation and management of preoperative anaemia in the elective general surgical patients.
Anaemia is the commonest haematological problem in the preoperative patients. Often it is a sign of underlying disease or associated co-morbid condition. A low haemoglobin level is associated with an excess risk in surgical procedure and its outcome. Therefore an understanding of any cause, evaluation and any potential treatment is crucial in the preoperative setting. Our objective was to determine prevalence, evaluation and management of preoperative anaemia in the elective general surgical patients. ⋯ Out of 830 patients during the study period, 125 patients were anaemic with haemoglobin level 2.1-9.9 g/dl. Fifty-six (44.8%) were male and 69 (55.2%) were female, and 68.5% patients were from rural areas. Hypochromic microcytic was seen in 101 (80.8%) cases. Commonest surgical problem associated with anaemia was malignancies (27.2%) followed by haemorrhoids (20%) and tuberculosis of abdomen (14.4%). Majority of patients (52%) were managed on oral iron supplements while 48% received blood transfusions.

Case ReportsPeripheral ossifying fibroma of oral cavity: histopathologic differential diagnoses.
Peripheral ossifying fibroma is a benign neoplasm that usuallydevelops from gingiva, presenting as an exophytic smooth surfaced pink or red nodular mass that is sessile or is less frequently seen on a pedicle. From the Indian perspective, it is usually noticed in 5th-6th decades of life with female predilection. ⋯ It has to be differentiated histopathologically from pyogenic granuloma, fibroma, peripheral giant cell granuloma, peripheral odontogenic fibroma and fibrous hyperplasia. A case of peripheral ossifying fibroma of maxillary gingiva in a 55-year-old Indian woman is reported.
