Case ReportsIntraoperative hyperventilation vs remifentanil during electrocorticography for epilepsy surgery - a case report.
Traditionally, intraoperative intracranial electroen-cephalography-recordings are limited to the detection of the irritative zone defined by interictal spikes. However, seizure patterns revealing the seizure onset zone are thought to give better localizing information, but are impractical due to the waiting time for spontaneous seizures. Therefore, provocation by seizure precipitants may be used with the precaution that spontaneous and provoked seizures may not be identical. ⋯ Hyperventilation and ultra short acting opioid remifentanil were used separately as intraoperative precipitatants of seizure patterns, while recording from subdural and intraventricular electrodes in a patient with temporal lobe epilepsy. Two different ictal onset zones appeared in response to hyperventilation and remifentanil. Both zones were resected and the patient has remained essentially seizure free for 1 year. Furthermore, this is the first description of hyperventilation used as an intraoperative seizure precipitant in human focal epilepsy.

Quantitative diffusion tensor imaging for evaluation of motor function in patients with brain infarcts.
To evaluate the quantitative diffusion value in ipsilateral cerebral peduncle of patients with differing motor function abilities and analyse the thresholds discriminating the poor motor function disability. ⋯ The ipsilateral FA value and FA ratio were feasible and better imaging indices in discriminating patients with poor motor function ability.

Review Case ReportsWhite matter disorders with autosomal dominant heredity: a review with personal clinical case studies and their MRI findings.
Leukoencephalopathies are a heterogeneous group of severe encephalopathy syndromes with myelin, axonal or vascular pathology, typically with extensive white matter lesions on MRI T2-FSE and/or -FLAIR sequences. ⋯ The diagnosis within the group of leukoencephalopathies thus characterized by MRI relies mainly upon clinical and genetic analysis. The differential diagnosis against treatable leukoencephalopathies is increasingly relevant.

Outcome prediction in traumatic brain injury: comparison of neurological status, CT findings, and blood levels of S100B and GFAP.
To investigate the predictive value of early serum levels of S100B and glial fibrillary acidic protein (GFAP) in traumatic brain injury. ⋯ Blood levels of S100B and GFAP indicate the severity of brain damage and are correlated with neurological prognosis after trauma. Both methods can yield additional prognostic information if combined with clinical and CCT findings.

Long-term levetiracetam treatment in patients with epilepsy: 3-year follow up.
To assess the long-term efficacy and tolerability of levetiracetam in routine clinical practice. ⋯ Levetiracetam is an effective and well-tolerated option for long-term treatment of epilepsy in adults.
