The Canadian Registry for Pulmonary Fibrosis: Design and Rationale of a National Pulmonary Fibrosis Registry.
Background. The relative rarity and diversity of fibrotic interstitial lung disease (ILD) have made it challenging to study these diseases in single-centre cohorts. Here we describe formation of a multicentre Canadian registry that is needed to describe the outcomes of fibrotic ILD and to enable detailed healthcare utilization analyses that will be the cornerstone for future healthcare planning. ⋯ Conclusion. CARE-PF will be among the largest prospective multicentre ILD registries in the world, providing detailed data on the natural history of fibrotic ILD and the healthcare resources used by these patients. As the largest and most comprehensive cohort of Canadian ILD patients, CARE-PF establishes a network for future clinical research and early phase clinical trials and provides a platform for translational and basic science research.

Facilitators and solutions for practicing optimal guided asthma self-management: the physician perspective.
To identify key solutions that facilitate the prescription of long-term asthma controller and provision of written self-management plans by physicians. ⋯ Physicians proposed multiple facilitators and solutions to support optimal practice, leading to the development of a novel taxonomy. Key suggestions varied across physician specialties and behaviours sought, emphasizing the need to carefully select the most promising knowledge translation interventions.

A comparison of work-exacerbated asthma cases from clinical and epidemiological settings.
Clinical and epidemiological studies commonly use different case definitions in different settings when investigating work-exacerbated asthma (WEA). These differences are likely to impact characteristics of the resulting WEA cases. ⋯ Findings from both types of studies should be considered when measuring the contribution of work to asthma exacerbations, identifying putative agents, and selecting industries and occupations in which to implement screening and surveillance programs.

Validity of chronic obstructive pulmonary disease diagnoses in a large administrative database.
Administrative databases are often used for research purposes, with minimal attention devoted to the validity of the included diagnoses. ⋯ The results support the routine ascertainment of the validity of diagnoses before using administrative databases in clinical and health services research.

Randomized Controlled Trial Multicenter Study
Impact of Candida species on clinical outcomes in patients with suspected ventilator-associated pneumonia.
The significance of Candida species in respiratory tract (RT) secretions in critically ill patients is unclear. ⋯ Patients with suspected VAP, in whom no bacterial pathogen was identified and in whom Candida species were isolated from RT cultures, exhibited a greater burden of illness compared with similar patients without Candida. Whether Candida species colonization of RT secretions is a marker of disease severity or actually contributes to poorer clinical outcomes remains unclear.
