[Critical care nursing and continuous renal replacement techniques in the Madrid Community].
The continuous renal replacement techniques (CRRT) aim to substitute the altered renal function during a period of time, presenting advantages compared to conventional hemodialysis (CH). This study aims to determine the situation of the CRRT in the Intensive care units (ICU) of the Madrid Community (MC) using a survey distributed to nurses (n = 131) of 14 ICU. It evaluates four aspects of the CRRT: management model, knowledge, problems and degree of satisfaction. ⋯ A total of 84.7% think that CRRT significantly increases the workloads and 62.6% believe that they should be done by the ICU nurses. The main problem is the lack of knowledge to resolve complications during the technique. We conclude that the ICU nursing can handle the CRRT, adapting the nurse/patient ratio; training programs should be established and the role of the teaching nurse defined in critical renal cares.

[Accumulated fluid balance in patients admitted to the ICU: is it really reliable?].
In most of the textbooks, it is considered that the balance calculated after admission and the losses measured and/or estimated is an inexact way of establishing the real balance. Thus daily monitoring of the weight variations is recommended as a single possible alternative. On the other hand, there are few studies that have strictly studied the reliability of the fluid balance calculated. ⋯ We find that, above all after the 6th day, the accumulated balance calculated adequately reflected the weight changes (mean error/day < 250 ml), regardless of the presence or not of fever, sweat, oral diet, feces or mechanical ventilation. When weight on admission to the ICU was less than 75 kg, the changes in the balance calculated adjusted even more to the weight change, the contrary occurring when the weight was greater than 75 kg. These findings suggest that the accumulated balance calculated represents a valid alternative to daily weighing of the patients and that factors such as body mass and/or surface should be taken into account to reach more exact estimations.

Comparative Study[Long term of quality of life and mortality in acute respiratory distress syndrome (ARDS) patients].
Although advanced technology and make many researches, the acute respiratory distress syndrome has been associated to high mortality. We prospectively evaluated the quality of life and mortality changes in patients with acute respiratory distress syndrome, was used quality of life indicators, as well as we determinated if positional strategie will be have an influence to falling-off in mortality. ⋯ Mortality from patients acute respiratory distress syndrome is described between ratio for others authors, as well as it doesn't condittioned for positional estrategie used. The quality of life of survivors have deteriorated moderately, it was attributable to chronic disease.

Review Comparative Study[Music therapy effectiveness to decrease anxiety in mechanically ventilated patients].
The aim of this review is to find out whether or not music therapy is an effective nursing intervention to decrease anxiety and promote relaxation in ventilator-dependent patients. For the purpose of this review, relaxation has been considered as a reduction in state anxiety and physiologic signs (heart rate, blood pressure or respiratory rate). A comprehensive search has been conducted in electronic databases (Cochrane Library, Medline, CINHAL, Embase and PsycLit) in order to identify systematic reviews on music therapy effectiveness or randomised control trials that compare the effectiveness of music therapy versus no music or other relaxation techniques in patients receiving ventilatory assistance. ⋯ Some methodological limitations make their results be not fully reliable and therefore, it has not been possible to reach a satisfactory answer. Further and more rigorous research is needed on this area, as there is not enough valid research to conclude that music therapy is an effective nursing intervention for decreasing patients' anxiety. As it causes no harm and is a relatively inexpensive intervention, it would be worth exploring its effects on different kind of outcomes and settings.

To demostrate that the use of water to seal facial mask for non invasive ventilation (NIV) reduces the incidence of facial pressure ulcers. ⋯ The use of water to seal facial mask during invasive non ventilation dealays the appearance of facial ulcers. More studies are needed to demostrate significant reduction in the incidence of ulcers.
