An Auxiliary Subxiphoid Incision in Nuss Procedure for the Treatment of Complex Pectus Excavatum.
An auxiliary subxiphoid incision was utilised to facilitate pectus bar placement and minimise operative risks in complex pectus excavatum. A series of 33 patients with recurrent or severe pectus excavatum underwent Nuss procedure with this incision from March 2013 to March 2016. The median age of the cohort was 6.9 years (9 females vs. 24 males). ⋯ Twenty-one cases had already the bars removed when this study was initiated. During follow-up (6-30 months), three cases presented with mild depression of lateral chest wall; while in the rest, the shapes of the chest wall remained satisfactory. The application of the auxiliary subxiphoid incision in Nuss procedure can make it safer in recurrent and severe pectus excavatum with favourable postoperative cosmetic effects.

Case ReportsUrosepsis caused by Kluyvera Ascorbata in a Pregnant Woman.
Among the Kluyvera species, K. ascorbata has been isolated from only a few adult cases. Furthermore, there is little or no information in the literature as to whether the species of Kluyvera can cause a clinically significant infection in pregnant women. We report a case of urosepsis caused by K. ascorbata in a 23-year pregnant woman at 26 weeks of gestation who presented with left flank pain. ⋯ Based on the clinical status of the patient and the antimicrobial susceptibility testing, the treatment was switched to ertapenem 1×1 g/day and was continued for 14 days. Among the Kluyvera species, K. ascorbata is the most frequent pathogen which may be isolated from pregnant women and can cause urosepsis. To the best of authors' knowledge, this is the first report showing the isolation of K. ascorbata in a pregnant woman which caused urosepsis.

Outcomes of Robot-assisted Laparoscopic Gynecological Surgery.
To compare the outcomes of robot-assisted (RA) and standard laparoscopic gynecological surgery (S-LGS) in a tertiary care hospital, and evaluate the factors affecting the outcomes of RA-LGS to identify areas of improvement. ⋯ There were no advantages of RA-LGS over S-LGS. Longer training periods for RA-LGS, with minimum 20-50 cases as part of a structured training programme, may improve outcomes.
