Post-operative pain (POP) is a form of acute, intense pain experienced in the period following surgery, whose adequate control is often problematic. This paper reviews determinants and characteristics of POP, together with rationale and current protocols for its management. ⋯ Acute post-surgical pain represents a crucial problem, but the multimodal therapeutic approach has enhanced the efficacy of pain-control while minimizing side-effects of each modality. Further improvement of POP control will necessarily involve better organization of pain services.

Co-prescription of gastroprotective agents in patients taking non-selective NSAIDs or COX-2 selective inhibitors: analysis of prescriptions.
To evaluate the prevalence of co-prescription of GPAs (proton pump inhibitors - PPIs and H2-receptor antagonists) and non-selective NSAIDs or COXIBs in patients registered under the local health authority (LHA) of Bergamo, a city in the north of Italy. ⋯ COXIBs seem to be used in patients at high risk of GI toxicity. However, the fear of GI adverse reactions and the uncertain safety profile of COXIBs leads many physicians to boost the gastroprotection by prescribing a PPI.

Randomized Controlled TrialA thorough QT/QTc study of multiple doses of tapentadol immediate release in healthy subjects.
This randomized, double-blind, placebo- and positive-controlled 4-way crossover study evaluated the effects of tapentadol immediate release (IR) on the QT/QTc interval. ⋯ Therapeutic and supratherapeutic doses of tapentadol do not affect the QT/QTc interval.

Time course of action and pharmacokinetics of ropivacaine in adult and elderly patients following combined lumbar plexus-sciatic nerve block.
Large doses of local anesthetics are needed in combined lumbar plexus-sciatic nerve block. The safety of local anesthetics is important among elderly patients. This study investigates the pharmacokinetics of ropivacaine in elderly patients with combined lumbar plexus-sciatic nerve block. ⋯ Age does not affect the onset time of ropivacaine after combined lumbar plexus-sciatic nerve block but can influence the motor and sensory recovery. Local absorption of ropivacaine is slower in elderly patients compared to adults.

Randomized Controlled Trial Comparative StudyRemifentanil in combination with ketamine versus remifentanil in spinal fusion surgery--a double blind study.
This study is aimed at conducting a program for two different anesthetic methods used during a spinal fusion surgery to ensure better intra-operative hemodynamic stability and post-operative pain control. ⋯ Adding low doses of ketamine hydrochloride could be a routine therapy to improve the hemodynamic stability and reduce the post-operative morphine consumption during spinal fusion surgery.
