Delirium is a serious complication of disorders of nervous system and requires early detection for successful treatment. Confusion assessment method for intensive care unit (CAM-ICU) is a fast and reliable tool for detection of delirium. We performed screening for delirium among patients of neurological ward using CAM-ICU. Our experience showed that CAM-ICU could be used for the identification of delirium in neurological patients excluding those who suffer from severe aphasia.

[Mild cognitive impairment in patients with coronary artery disease: clinical-demographyc and EEG-correlates].
The clinical and demographic factors associated with mild cognitive impairment (MCI) and the topographical features of electroencephalogram (EEG) spectral power in patients with coronary artery disease (CAD) depending on the presence or absence of MCI were studied. ⋯ Education is an important factor to preserve cognitive abilities in patients with CAD. In addition, the severity of coronary artery lesions and low cardiac contractility affect the degree of cerebral ischemia and neuronal dysfunction detected by spectral EEG power. The efficacy of quantitative EEG analysis methods for early detection and prevention of cognitive disorders in CAD patients was confirmed.

[The possibilities of increasing of efficacy of treatment in patients with dorsalgia].
An influence of L-lysine aescinat on decrease of pain syndrome and normalization of movement's functions. MATERIAL AND METHODS. 220 patients with dorsalgia has been studied. To asses treatment efficacy the Huskisson's visual analog scale, multidimensional verbal and color test of pain and the modified Waddel's scale were used. ⋯ The results of the study revealed that the use of L-lysine aescinat promotes considerable acceleration of process of regression of a pain syndrome and the efficacious rehabilitation of movements and plays a significant role in faster and expressed knocking over of a radicular syndrome.

To study pathogenetic features of chronic joint pain, we examined 183 patients with rheumatoid arthritis (RA) and 80 patients with osteoarthrosis (OA). The presence of mixed pain syndrome was found. A neuropathic component of pain (NCP) was observed in some patients with nociceptive pain (43 and 30% with RA and OA, respectively). ⋯ No signs of the damage of the somatosensory nervous system were found in patients with OA. Neuropathic pain was concomitant to secondary hyperalgesia which covered the zones localized far from the affected joint that allowed to suggest the involvement of dysfunctional mechanisms in the pathogenesis of chronic pain in OA. The study opens new possibilities for pharmacotherapy.

Comparative Study[Delirium in the acute phase of stroke: frequency and predisposing factors].
Delirium is a common complication of stroke worsening its prognosis. In this work we have analyzed the frequency of delirium and factors, predisposing to its development in the population of Russian patients in hyperacute phase of stroke. ⋯ Patients of older age, those with severe stroke, with prominent chronic cerebral changes on brain CAT scan, with fever, those with urine catheter and with positive snout reflex were more likely to develop delirium. Severity of posterior leucoareosis was the only independent predictor of delirium according to multivariate analysis.
