Randomized Controlled TrialEffects of combined lipoic acid and pyridoxine on albuminuria, advanced glycation end-products, and blood pressure in diabetic nephropathy.
This study was designed to investigate the effects of combined administration of lipoic acid and pyridoxine on albuminuria, oxidative stress, blood pressure, serum advanced glycation end-products, nitric oxide (NO), and endothelin-1 in patients with diabetic nephropathy. Thirty-four patients were randomly assigned to either a supplement group or a placebo group. The patients in the supplement group received 800 mg lipoic acid and 80 mg pyridoxine daily for 12 weeks, whereas the placebo group received corresponding placebos. ⋯ No statistically significant differences were observed between the two groups in mean changes of serum endothelin-1, glucose, and diastolic blood pressure. The present study indicates that combined administration of lipoic acid and pyridoxine improves albuminuria in patients with diabetic nephropathy by reducing oxidative stress, advanced glycation end-products, and systolic blood pressure. The reduction in microalbuminuria may be of benefit in retarding the progression of diabetic nephropathy.

Randomized Controlled TrialDASH-like diets high in protein or monounsaturated fats improve metabolic syndrome and calculated vascular risk.
Weight-loss diets with varying proportions of macronutrients have had varying effects on weight loss, and components of metabolic syndrome and risk factors for vascular diseases. However, little work has examined the effect of weight-neutral dietary changes in macronutrients on these factors. This is an investigation using the OMNI Heart datasets available from the NHLBI BioLINCC program. ⋯ All 3 diets lowered a calculated 10-year risk of cardiovascular disease, with the high-protein and unsaturated fat diet being the most efficacious (p < 0.001). Only the unsaturated fat diet showed a slightly decreased calculated 9-year risk of diabetes (p = 0.11). Of the 3 weight-neutral diets, those high in protein and unsaturated fats appeared partially or wholly most beneficial.

Randomized Controlled TrialEffects of mixed dietary supplements on total plasma homocysteine concentrations (tHcy): a randomized, double-blind, placebo-controlled trial.
Although a number of studies have reported raised total plasma homocysteine (tHcy) concentrations in free-living older people, there are no data on homocysteine response to a mixed nutrient supplement in older patients. A raised plasma homocysteine concentration in older patients is partly a reflection of their co-morbidity, including impaired renal function, and there is uncertainty about the extent to which dietary interventions can improve plasma tHcy. ⋯ A mixed nutrient supplement containing physiological amounts of B vitamins significantly reduced plasma tHcy concentrations in older patients recovering from acute illness.

Randomized Controlled Trial Clinical TrialEffects of 3-week consumption of green tea extracts on whole-body metabolism during cycling exercise in endurance-trained men.
The consumption of green tea has been generally associated with beneficial effects on human whole-body metabolism and recent investigations with animals indicate favorable effects of green tea extracts (GTE) on energy metabolism during exercise and aerobic exercise performance. Therefore, the purpose of this study was to examine the effects of a three-week supplementation with GTE on human energy metabolism during submaximal cycling exercise. ⋯ The GTE supplementation did not influence indices of fat and energy metabolism (fatty acids, 3-beta-hydroxybutyrate, triacylglycerol, low-density-lipoprotein cholesterol, total cholesterol, lactate, glucose, oxygen uptake, respiratory exchange ratio, energy expenditure), inflammation processes (interleukin-6, C-reactive protein), and oxidative stress (thiobarbituric-acid reactive substances, oxidized low-density-lipoprotein cholesterol), but plasma creatine kinase concentration at rest and during exercise was significantly lower (p = 0.039) and high-density-lipoprotein cholesterol concentration at rest was significantly higher (p = 0.043) compared to placebo. In conclusion, these results suggest only slight effects on whole-body metabolism after supplementation with GTE.
