Stereotactic radiosurgery, known for efficacy in the noninvasive treatment of tumors or arteriovenous malformations that are otherwise difficult to access through traditional surgery, is an emerging technology in the treatment of focal epileptic lesions. Recent studies in the treatment of hypothalamic hamartoma and mesial temporal lobe epilepsy further suggest that seizures in these medically intractable syndromes remit at clinically significant rates with stereotactic radiosurgery. Further studies will be required to determine whether efficacy attains that of traditional surgery while offering a noninvasive technique with potentially lower morbidity.

Comparative StudyVoxel-based assessment of differences in damage and distribution of white matter lesions between patients with primary progressive and relapsing-remitting multiple sclerosis.
Several studies have reported lower focal demyelination and inflammatory activity in primary progressive multiple sclerosis (PPMS) than in relapsing-remitting MS (RRMS). However, very little is known about possible differences in damage and distribution that may occur within lesions visible on magnetic resonance imaging in the 2 forms of the disease. ⋯ Differences in cerebral pathologic involvement exist between RRMS and PPMS and contribute to variations in clinical disability.

Effect of neurologic complications on outcome after heart transplant.
To study neurologic complications after heart transplant. ⋯ Perioperative neurologic complications are frequent in heart transplant recipients, but most are transient and inconsequential. However, perioperative stroke is the most important neurologic complication affecting survival in the first year after heart transplant. Infectious diseases of the central nervous system are associated with fatal outcome.

Clinical spectrum of reversible posterior leukoencephalopathy syndrome.
Reversible posterior leukoencephalopathy syndrome (RPLS) is characterized by neuroimaging findings of reversible vasogenic subcortical edema without infarction. The clinical syndrome of RPLS typically involves headache, encephalopathy, visual symptoms, and seizures. ⋯ This is the largest clinical series to date of RPLS with confirmed neuroimaging improvement. Clinical recovery occurred in most patients within days. The condition was rarely isolated to the parieto-occipital white matter, and atypical neuroimaging features were frequent.

Clinical TrialCombination therapy with interferon beta-1a and doxycycline in multiple sclerosis: an open-label trial.
To evaluate the efficacy, safety, and tolerability of combination therapy with intramuscular interferon beta-1a and oral doxycycline, a potent inhibitor of matrix metalloproteinases, in patients with relapsing-remitting multiple sclerosis (RRMS) having breakthrough disease activity. ⋯ Combination of intramuscular interferon beta-1a and oral doxycycline treatment was effective, safe, and well tolerated. Controlled clinical trials in larger cohorts of patients with MS are needed to evaluate the efficacy and tolerability of this combination. Trial Registration clinicaltrials.gov Identifier: NCT00246324
