Phosphine gas constitutes a potential and serious little-known cause of poisoning of professional nature of the medical staff and nursing care of patients who voluntarily swallow phosphides rodenticides purposes suicide. The objective of this paper is to inform to healthcare workers from urgencies, forensic and occupational health services on this occupational hazard. We present the case of a nurse who suffered from poisoning by gas phosphine confirmed through an environmental monitoring of gases in an emergency department carried out by the government service of civil protection of the State of Jalisco.

Uterine fibroids are a significant source of morbidity for women of reproductive age, and can result in substantial symptoms affecting their quality of life. Definitive treatment has traditionally been a myomectomy, but increasingly women are not prepared to undergo such an invasive procedure for a benign and usually self-limiting condition. ⋯ Magnetic resonance guided focused ultrasound (MRgFUS) ablation combines therapy delivered by an ultrasound transducer with imaging guidance for therapy and thermal feedback provided by magnetic resonance imaging. This paper provides an overview of the MRgFUS technique, including a brief description of the treatment system, guidelines for selection of patients and follow-up, and a comparison with other treatment techniques.

Retirement is a time of life marked by many social prejudices. However, links of retirement to depression do not have any scientific evidence. Retired physicians do not suffer more psychiatric disorders than those who continue working. ⋯ Most research that has studied the quality of life of retired physicians agrees that most doctors are satisfied with their new condition. Similarly, retired doctors who said they were dissatisfied with their new situation, this was not because they had left the profession, but because they were in poor health or had no family or economic stability. The present study aims at providing an overview of occupational retirement from the individual and ontological point of view.

Cytomegalovirus (CMV) is a β herpesvirus and a significant human pathogen. After primary infection establishes life long latency. In immunocompetent individuals cell-mediated host immune responses prevent the development of overt CMV disease. ⋯ In the pathogenesis are involved immunodysfunction and imbalance in immunomodulatory mediators principally tumor necrosis factor (TNF) and nuclear factor κB (NF-κB). Several studies have shown an association between CMV reactivation in immunocompetent critically ill patients and poor clinical outcomes. Further studies are warranted to identify subsets of patients who are at risk of developing CMV reactivation and to determine the role of antiviral agents on clinically outcomes in critically ill patients.

In this paper we review the main medical programs from the Universidad Nacional Autonoma de Mexico (UNAM) faculty of medicine in the first seventy-five years of twentieth century. We have selected those instances that are representative of special moments in the development of Mexican medical knowledge, and also corresponding to the need to prepare medical students in view of social demands. ⋯ After those, the 1956 and 1960 programs are mentioned and briefly analyzed; both were founded on the concern to develop a clinical reasoning in the student. Finally, the 1974 Integral General Medicine Program directed its interest to social and environmental themes.
