Comparative Study
[Accidental or non-accidental brain injury in infants. Prospective study of 88 cases].
To study the epidemiology of head injury (HI) in infants, the factors favouring the occurrence of a subdural haematoma (SDH), the prevalence of retinal haemorrhages (RH) and the prognostic factors, by comparing the non-accidental (NAHI) and accidental (AHI) head injuries. RH, in particular, are of fundamental value in the diagnosis of NAHI but, in the absence of systematic studies, their sensitivity and specificity for the diagnosis of the NAHI have rarely been assessed. ⋯ Although only representing 33% of cases, child abuse was responsible for 2/3 of the deaths and for the totality of the severe morbidity in our series. The infants exhibiting perinatal problems represented an important group at risk of abuse, which justified their regular medical-social follow-up.

Human metapneumovirus (hMPV) is a newly identified virus responsible for occasionally severe respiratory infections, mainly observed in children. ⋯ hMPV testing has clinical utility in adult patients with acute respiratory tract infection.

Varicella, an almost compulsory affection in children, is complicated in 2% of cases. Some treatments such as powders and non-steroidial anti-inflammatories (NSAIs) may favorise or worsen infectious complications. ⋯ It is crucial that practitioners be informed on the treatment of varicella and the risks of severe cutaneous surinfections enhanced by certain prescriptions.

Randomized Controlled Trial Clinical Trial
[Proof of the pharmacodynamic activity of Flector Plaster in healthy volunteers and patients suffering from osteo-arthritis of the knee].
STUDY OF HEALTHY VOLUNTEERS: This double blind study (bioadhesive plaster of diclofenac epolamine versus placebo), showed that the application of diclofenac epolamine for 24 hours induced a selective hypoanalgesic effect on the muscle, proportional to the degree of hypersensitivity to the pain observed in baseline conditions. The enhanced effect on the hyperalgic muscle can be explained by the direct impact of the active ingredient on the nociceptors and/or its inhibiting effect on the local production of prostaglandins. STUDY OF PATIENTS SUFFERING FROM BILATERAL OSTEO-ARTHRITIS OF THE KNEE: In a 24 hour trial of the treatment (bioadhesive plaster impregnated with diclofenac epolamine (Flector Plaster) versus placebo), the active treatment had a significant impact on the spontaneous pain during rest that was enhanced during mouvement. In a longer term trial (72 hours), the regression of the spontaneous pain on the visual analog scale appeared even more significant, still lasting 96 hours after withdrawal of the plaster, not only at rest but also on mouvement.
