Therapeutic Effects of Bipolar Coagulation Forceps on Open Thyroid Surgery.
The aim was to compare the therapeutic effects of bipolar coagulation forceps, harmonic scalpel, and conventional thyroidectomy on open thyroid surgery. ⋯ The effects of bipolar coagulation forceps on open thyroid surgery exceeded those of the harmonic scalpel and conventional thyroidectomy. This method is worthy of promotion in clinical practice.

[Decrease of liver fat content by aerobic exercise or metformin therapy in overweight or obese women].
Nonalcoholic fatty liver disease (NAFLD) is characterized by high intrahepatic triglyceride level. It is associated with an increased prevalence of cardiovascular disease, independently of underlying cardiometabolic risk factors. Metformin is used as a pharmacological treatment; the adherence is low because of the presence of adverse effects. Aerobic exercise could be an alternative therapy, but its effectiveness compared with metformin has not been established in the treatment of NAFLD. The aim of this study was to establish the effectiveness of aerobic exercise and its influence in reducing cardiovascular risk in overweight or obese women with NAFLD. ⋯ An aerobic exercise program of 12 weeks with a volume training of 300 min/week and a moderate to vigorous intensity (60-85% VO2peak) modified the liver fat content and improved cardiovascular risk factors during the intervention. Exercise contributed to a holistic approach by modifying a number of the components of metabolic syndrome, cardiorespiratory-fitness, and cardiovascular risk.

Aldosterone receptor antagonists induce favorable cardiac remodeling in diastolic heart failure patients.
Serum levels of aldosterone in heart failure are increased up to 20 times compared to normal subjects. After an acute myocardial infarction, aldosterone increases progressively as well as interstitial fibrosis and collagen synthesis from cardiac fibroblasts, forming a patchy heterogeneous interstitial collagen matrix that affects ventricular function. Even if angiotensin converting enzyme inhibitors (ACEI) or angiotensin II receptor antagonists (ARA) can reduce aldosterone levels early during treatment, they increase again after a 12 week treatment. The aim of this study was to evaluate the changes in structure and function of the left ventricle in symptomatic (NYHA I-III) diastolic heart failure patients receiving an aldosterone receptor antagonist. ⋯ Aldosterone receptor antagonists reduce or avoid increasing of PSAP and inducing a favorable remodeling of the left ventricle, especially in the IVS in diastolic heart failure patients.

[Decrease of folic acid and cognitive alterations in patients with epilepsy treated with phenytoin or carbamazepine, pilot study].
Phenytoin and carbamazepine were the antiepileptic drugs most frequently used in Mexico and throughout the world. Epileptic patients who take these drugs have a variety of collateral effects including the decrease of folates plasmatic level. Low seric folic acid concentration has been associated with a decline in cognitive functions. The administration of a combined treatment with folic acid could ameliorate these difficulties. ⋯ Treatment with folic acid is safe and without side effects, it improved the cognitive function in patients with epilepsy treated with phenytoin and carbamazepine.

[Postoperative analgesia using epidural administration of bupivacaine plus lidocaine].
To compare the postoperative analgesia of bupivacaine + lidocaine versus lidocaine in lower abdominal surgery. ⋯ The mixture of bupivacaine + lidocaine proved be an effective alternative for epidural use in lower abdominal surgery.
