The administration of parenteral infusates is a frequent intervention that is considered innocuous; moreover, the risk of this procedure which offers a direct access to the bloodstream is minimized. ⋯ The strategies established to avoid the contamination of parenteral infusions were associated with a reduction in the proportion of gram-negative rods in blood cultures, although the proportion is still higher than that in developed countries, probably related to catheter contamination. We suggest establishing similar strategies in other hospitals from developing countries.

Diabetes mellitus (DM) is a highly prevalent disease in the geriatric population. The elderly are at increased risk for adverse drug reactions (hypoglycemia) compared with the young due to physiological changes associated with age, high number of comorbidities and polypharmacy usually present, causing changes in drugs pharmacokinetics and pharmacodynamics. At present, there are wide therapeutic options to treat DM, so it is important to know the different drug families and their potential side effects. ⋯ About the new drugs that act on the DPP-4, results are promising with a low risk profile of adverse reactions. We must consider the use of human insulin or insulin analogues in the elderly when not achieved with other hypoglycaemic agents. At the start of therapy is important to establish the most appropriate management goals based on patient comorbidities and their life expectancy.

We review important aspects of the pandemic influenza A (H1N1) at the time of declaring the end of the contingency in Mexico. The pre-established surveillance system had to be modified during the course of the epidemic. From the first epidemic weeks, viral monitoring recorded the displacement of other pathogens by the pandemic virus. ⋯ The difficulties of surging services for seriously ill patients are described. Preventive measures such as the use of masks and hand hygiene are reviewed, as well as the vaccination drive and the difficulties for its application in health personnel. The review concludes with the need to learn the teachings of the pandemic, describing the necessary elements to prepare against the next one.

To determine the efficacy of sedation in outpatient procedures performed by Pediatric Residents. ⋯ The procedures were done successfully; the pediatric residents demonstrated the required preparation for sedating and reverting adverse effects.
