Randomized Controlled TrialEvaluation of the postoperative cognitive dysfunction in elderly patients with general anesthesia.
The present study is aimed to study the neuron-specific enolase (NSE) and S100b proteins in the evaluation of postoperative cognitive dysfunction in elderly patients with general anesthesia. ⋯ The expression levels of S100b protein and plasma NSE in the serum of POCD patients increased, which indicated the severity of the disease. The preoperative intramuscular injection of scopolamine butylbromide has important clinical significance for the prevention of POCD.

ReviewAnesthesia and analgesia in breast surgery: the benefits of peripheral nerve block.
Breast surgery is frequently associated with postoperative pain, nausea and vomiting, that result in increased patient's suffering, prolongation of hospital stays and related costs. Thoracic paravertebral nerve block (TPVB) is a viable option to the classic multimodal analgesia in breast surgery as it enhances surgical anesthesia and postoperative analgesia. In this review, we report the results of a number of studies on the role of TPVB in breast surgery. ⋯ In particular, TPVB seems to provide the most benefits in patients undergoing an unilateral or bilateral mastectomy followed by immediate reconstruction. Some studies also suggest that the use of regional anesthesia-analgesia could attenuate perioperative immunosuppression and minimize metastases in breast cancer patients. TPVB can be also coupled with other regional anesthetic techniques such as pectoral nerve block (PNB), thus increasing the reduction in postsurgical pain, opioids consumption and length of hospital stays.

Observational StudyRelationship of serum novel adipokine chemerin levels with body composition, insulin resistance, dyslipidemia and diabesity in Saudi women.
Chemerin is believed to be a mediator for the adipose tissue inflammation that occurs in obesity. The present study compared chemerin levels between healthy and type 2 diabetic women matched for age and body composition. We also aimed to assess the relationship of serum chemerin levels with body composition, insulin resistance, dyslipidemia, and diabesity. ⋯ Serum chemerin levels are elevated in patients with type 2 DM compared to healthy control subjects and are positively correlated with adiposity and Insulin resistance in patients with type 2 DM.

The role of miR-146a in modulating TRAF6-induced inflammation during lupus nephritis.
Lupus nephritis (LN) is a major complication of systemic lupus erythematosus (SLE). A previous study showed decreased expression level of microRNA (miR)-146a in LN patients, indicating its possible role in LN pathogenesis. ⋯ The expression of miR-146a in renal tissues of LN patients was significantly depressed, while the transcriptional activity of TRAF6 and NF-κB was enhanced. MiR-146 thus inhibited NF-κB transcriptional activity and inflammatory factor synthesis, and alleviated chemotactic effect towards macrophage via the inhibition of TRAF6 activity.
