Clinical Trial[Treatment of chronic myeloid leukemia with imatinib in the accelerated stage of the disease].
Philadelphia chromosome positive chronic myeloid leukemia (Ph+ CML), in advanced stage of disease, is resistant to standard chemotherapy. Imatinib was found to be effective in these patients. This paper shows our preliminary results. ⋯ After a 9-month follow up Ph+ CML progressed in 9 patients (60%) and 4 of them died. The most frequent adverse effects were edema, nausea, neutropenia and thrombocytopenia. Imatinib mesylate has a substantial, but short term activity in the accelerated phase and blastic crisis of the Ph+ CML.

[Recirculation of urea and dialysis efficiency using dual-lumen dialysis catheters in various locations: may the venous lumen of the catheter be used as the arterial lumen and vice versa?].
To measure percentage of urea recirculation in hemodialysis by a dual-lumen central venous catheters of various localisations (e.g. jugular, subclavian and femoral), and also in cases when the venous lumen of the catheter is used as arterial lumen, and vice versa, and under these conditions to evaluate the efficiency of hemodialysis based on the measurements of urea reduction ratio. ⋯ When hemodialysis is delivered by a correct blood lines connection the measurements have shown a decrease in urea recirculation by 5% for jugular and subclavian catheters, and 5%-10% for femoral catheters. However, in cases when the venous lumen of the catheter is used as an arterial lumen, and vice versa, urea recirculation is below 10% for jugular catheters; whereas in femoral catheters the percentage is higher than 20%. Despite so high percentage of urea recirculation, that we obtained for such use of the catheters, urea reduction ratio in hemodialysis via jugular and subclavian catheters is > 60%, whereas via femoral catheters, the percentage is significantly lower. Consequently, the efficiency of hemodialysis is reduced, and such use of femoral catheters should therefore be avoided.

Historical Article[100 years of continuous preventive medicine in the area of Rijeka].
The Hungarian Ministry of Agriculture established a chemical investigation service named. The Hungarian Royal Experimental Chemical Service of the City of Rijeka. According to the information found in the city's archive, as well as to the news published in the daily newspaper of that time "La Bilancia", the Service started its activity on April 8th, 1900. ⋯ The Hygienic Institute and the Health Care Center Rijeka (founded in 1953) were unified into the Institute of Public Health on July 1st 1960. A huge improvement in preventive medical service was obtained by moving into a new building of the Institute in nowdays location in Kresimirova St. No 52a.

Historical Article[80 years of hypophyseal surgery at the Sestre Milosrdnice Hospital].
Sestre milosrdnice University Hospital has celebrated its 155th anniversary. In this period, it has been the origin of Croatian medical masterminds and a place of numerous pioneer events. After V. ⋯ Since 1982 until today Z. Gnjidić has performed surgery on more than 1000 patients using transsphenoidal approach. The above mentioned results belong to the top ten world series.

The reform of government financing of health services has caused a lot of significant changes in hospitals. The main points of the reform are limited hospital expenses, and reduced financial calculation. The following three factors are influencing upon hospital expenses is the only factor that hospital management can effectively influence upon. ⋯ Other ones are staff expenses, that include improvement of working process and adaptable time-table. Pharmaco-economical evaluation of the new drugs has shown how analysis of expenses structure could be acceptable in the scope of anesthesiology. Because of the permanent deficit of financial resources, permanent decentralized control is indispensable.
