The risk of congenital malformations is two to three times higher among women with diabetes mellitus before pregnancy. Gestational diabetes is emerging as a risk factor for malformations. ⋯ Pregnant women with pre-existing or gestational diabetes have a higher risk of giving birth a child with malformations.

Case Reports[Decompressive hemicraniectomy for malignant middle cerebral artery infarction. Report of two cases].
Malignant middle cerebral territory infarction represents 5 to 10% of all brain infarctions. Its mortality is 80%, due to brain herniation and it is not reduced by medical treatment. ⋯ We report two male patients aged 61 and 54 years, with a malignant middle cerebral territory infarction who were treated with decompressive hemicraniectomy. After two years of follow up, both patients are self-sufficient and live at home with their families.

The mortality of grade V and VI liver trauma fluctuates between 30% and 70%. The atriocaval shunt, described by Shrock et al, in 1968, is a therapeutic option that, after being installed, allows to repair the suprahepatic veins and retrohepatic cava in a bloodless surgical field. Its use requires an experienced and skilled surgeon to obtain survival rates similar to those obtained with other methods. We report two male patients of 17 and 18 years old treated successfully with this technique after suffering a blunt and a penetrating liver trauma by a shotgun, respectively.

[Critical appraisal: enteral nutrition is better than parenteral nutrition for patients with acute pancreatitis].
To compare the safety and clinical outcomes of enteral and parenteral nutrition in patients with acute pancreatitis. ⋯ Enteral nutrition should be the preferred route of nutritional support in patients with acute pancreatitis.
