Review Case Reports[Intracardiac thrombosis in a patient with primary antiphospholipid syndrome. Report of a case].
Antiphospholipid antibody syndrome is characterized by arterial or venous thrombosis associated to the presence of antiphospholipid antibodies. We report a 32 atrial thrombus. ⋯ The laboratory work-up confirmed an antiphospholipid syndrome. Five months later the patient has not presented new thrombotic episodes and is receiving oral anticoagulants and antiplatelet therapy.

Biography Historical Article[1896-1996: a century of cardiac surgery. Ludwig Rehn and the first heart surgery].
In 1896 Stephen Paget published his book "Surgery of the Chest", where he affirmed that heart surgery had probably reached the limits of nature for any kind of surgery. This statement followed Billroth's who, few years earlier, sustained that the surgeon who attempted a suture of a wound in the heart was sure to loose forever the consideration of his colleagues. However, that very year of 1896 in Frankfurt, Ludwig Rehn succeeded in suturing a stab wound of a man's heart. By accomplishing this, he not only proved his colleagues wrong, but he made the heart, center of life and site of the soul, accessible to the surgeon, opening the way to what is today cardiovascular surgery.

Multicenter Study Clinical Trial[Tropisetron for the prevention of nausea and vomiting during chemotherapy: multicenter clinical study].
The antiemetic effect of tropisetron was studied in 97 cancer patients (67 men, 30 women) receiving cisplatin in doses of 75 mg/m2 or higher. On 279 chemotherapy cycles studied (max 6 per patient) 5 mg of tropisetron was administered once a day i.v on day 1 and p.o. on days 2 to 6. Efficacy preventing vomiting and nausea was measured in 24 hour period as: complete control O episodes, major control 1 to 2 episodes, minor control 3 to 4 episodes and no control 5 or more episodes. ⋯ We conclude that tropisetron allows satisfactory control of acute and delayed vomiting in a high percentage of patients treated with high doses of cisplatin. The drug does not have significant secondary effects. Tropisetron administration in only one daily dose implies an evident advantage and a treatment cost reduction.

Clinical Trial[Predictor indices of early extubation in mechanical ventilation in patients treated with heart surgery].
Following open heart surgery, most patients are ventilated for 12 to 24 hours to obtain a period of hemodynamic stability and to reduce the work of breathing. Some authors have proposed criteria to guide early extubation and have proposed physiologic parameters to predict which patients will be able to breathe spontaneously. ⋯ Pulmonary function parameters and blood gases measured during a T piece trial are not good predictors of early extubation in cardiac surgery.

Clinical Trial[Felodipine in the treatment of essential arterial hypertension in elderly patients].
Felodipine is a calcium channel inhibitor with high vascular selectivity. ⋯ Felodipine is effective in the treatment of elders with essential hypertension.
