Assessment of in-hospital cardiopulmonary resuscitation using Utstein template in a university hospital.
The aim of this study was to evaluate the effectiveness of in-hospital cardiopulmonary resuscitation (CPR) strategies and identify key predictors of post-CPR survival in a university hospital setting. Using a form recommended by the European Resuscitation Council, data regarding in-hospital CPR attempts from January 2001 to December 2002 were recorded and analyzed. The main outcomes of interest were immediate survival after CPR and survival to hospital discharge. ⋯ The key predictors of survival to hospital discharge were CPR duration, immediate defibrillation, Glasgow Coma Scale score, and Early Prediction Score. Together, our results suggest that in-hospital CPR strategies require improvement. They also underscore the importance of data collection and analysis in evaluating the effectiveness of inhospital CPR strategies.

The clinical, radiological and surgical characteristics of supratentorial penetrating craniocerebral injuries: a retrospective clinical study.
Penetrating craniocerebral injuries (PCIs) are the most fatal injuries of the head that usually have a poor outcome. From the parenchymal destructions to ventricular lacerations, a wide variety of damages occurs during the injury. Surgical treatment is still the mainstay of the management in these patients. ⋯ Shrapnel and bullet were the most wounding agents. All of the patients underwent surgical treatment following clinical and radiological evaluations. Nine of them were died and 7 were rehabilitated because of severe neurological deficits.

Randomized Controlled Trial Comparative Study Clinical TrialComparison of propofol and sevoflurane for laryngeal mask airway insertion.
In this study, we compared haemodynamic changes, laryngeal mask airway (LMA) insertion time, and complications in patients anesthetized by inhalation of sevoflurane with those of intravenous induction with propofol. One hundred patients, aged between 20-40 years were enrolled in this study. Group 1 received propofol (2.5 mg/kg i.v.) and group 2 received sevoflurane (6%+50% N2O+50% O2) by inhalation using the tidal volume technique. ⋯ Apnoea was significantly higher in propofol group (40%) than in sevoflurane group (0%). Sevoflurane is an alternative to propofol for induction of anaesthesia and has a lower incidence of apnoea. Other complication rates are not higher than which propofol but the longer duration of induction time is a disadvantage.

Effects of ketamine and propofol on the ratio of interleukin-6 to interleukin-10 during endotoxemia in rats.
Our previous study reported that the change in the ratio of interleukin (IL)-6 to IL-10 influences the severity of sepsis in patients with systemic inflammatory response syndrome. We evaluated the change in the ratio of IL-6 to IL-10 after administration of ketamine or propofol in endotoxin-exposed rats in order to evaluate the relationship of pro-inflammatory and anti-inflammatory cytokines following ketamine or propofol administration during endotoxemia. ⋯ The current study showed that ketamine and propofol administration attenuated the increase in TNF-alpha, IL-6, and IL-10, and ketamine attenuated the increase in the ratio of IL-6 to IL-10, but propofol increased this ratio in rats receiving a single intravenous bolus of endotoxin. While the mechanisms responsible for the inhibitory effects require further investigation, our results suggest that proper use of ketamine as an anesthetic agent may offer certain advantages in the management of patients with endotoxemia.

This study was designed to investigate the oxidative stress parameters in laryngeal cancer and cancer-free adjacent tissues. Lipid peroxidation end product and the endogenous antioxidant components-CuZn superoxide dismutase (CuZn SOD) glutathione peroxidase (GSH Px), glutathione reductase (GSSG Rd) and glutathione (GSH)-were analysed by spectrophotometric and kinetic methods. ⋯ CuZn SOD and GSH Px activities and GSH level were significantly higher and GSSG Rd activity significantly lower in the cancer tissue. Detection of the antioxidant status may be useful to determine the tumour resistance to therapy, to choose the correct radiotherapy/chemotherapy and to monitor the effectiveness of the therapetic strategy.
