A randomized controlled trial of endoscopist vs. anaesthetist-administered sedation for colonoscopy.
Endoscopist-administered propofol sedation for colonoscopy has not been compared to anaesthetist-administered deep sedation in clinical trials. Our aim was to compare patients' satisfaction and safety during these two sedation modalities. ⋯ Endoscopist-administered propofol sedation for colonoscopy offered a better level of satisfaction and fewer side-effects than anaesthetist-administered deep sedation.

The management of vitamin K antagonists (VKAs) associated coagulopathy is a cornerstone of pre-endoscopic assessment of anticoagulated patients presenting with acute gastrointestinal bleeding. ⋯ There is a considerable variability amongst physicians in the management of gastrointestinal bleeding patients with VKAs-associated coagulopathy and a poor compliance to practice guidelines. These data indicate that better education is needed in this area.

Over the past few decades, advancements in minimally invasive surgery techniques have made surgical management of gastroesophageal reflux diseases increasingly popular. More recently, the field of minimally invasive surgery has experienced a natural evolution towards a reduction in the invasiveness of surgery and even in the number of abdominal access incisions. ⋯ However, there is very little published data on the application of this technique in anti-reflux surgery. We present a brief review of available data as well as a summary of our experiences with this innovative approach to minimally invasive foregut surgery.
