Association between tracheostomy timing and outcomes for older critically ill COVID-19 patients: prospective observational study in European intensive care units.
Tracheostomy is performed in patients expected to require prolonged mechanical ventilation, but to date optimal timing of tracheostomy has not been established. The evidence concerning tracheostomy in COVID-19 patients is particularly scarce. We aimed to describe the relationship between early tracheostomy (≤10 days since intubation) and outcomes for patients with COVID-19. ⋯ ClinicalTrials.gov NCT04321265.

Multicenter Study Observational StudySingle Nucleotide Variant in FAS Associates With Organ Failure and Soluble Fas Cell Surface Death Receptor in Critical Illness.
Multiple organ failure in critically ill patients is associated with poor prognosis, but biomarkers contributory to pathogenesis are unknown. Previous studies support a role for Fas cell surface death receptor (Fas)-mediated apoptosis in organ dysfunction. Our objectives were to test for associations between soluble Fas and multiple organ failure, identify protein quantitative trait loci, and determine associations between genetic variants and multiple organ failure. ⋯ We found that single nucleotide variant rs982764G was associated with lower plasma soluble Fas concentrations in a discovery and validation population, and single nucleotide variant rs982764G was also associated with lower organ dysfunction on day 3. These findings support further study of the Fas pathway as a potential mediator of organ dysfunction in critically ill patients.

Neuromuscular Blocker Use in critically Ill Children: Assessing Mortality Risk by Propensity Score-Weighted Analysis.
We aim to describe the use of continuous infusion of neuromuscular blocking agents in mechanically ventilated critically ill children and to test its association with in-hospital mortality. ⋯ Thirteen percent of mechanically ventilated children in PICUs received neuromuscular blocking agents. When adjusting for selection bias with a propensity score approach, the use of neuromuscular blocking agent was found to be a significant predictor of in-hospital mortality.

Micronutrient intake from enteral nutrition in critically ill adult patients: A retrospective observational study.
The objective of this study was to determine the intake of micronutrients including vitamins B12, D, C, and A; folate; thiamine; iron; zinc; and selenium that are delivered from enteral nutrition (EN) during routine clinical practice in critically ill adults, expressed as a percentage of the Australia and New Zealand nutrient reference values. ⋯ EN delivery met the recommended intake for four micronutrients, did not meet the recommended intake for five micronutrients, and did not exceed the upper level of intake for any micronutrient when approximately 50% energy adequacy was achieved types.
