Myasthenia gravis (MG) affects the ocular, bulbar, and proximal limb muscles. The involvement of distal limb muscles is uncommon. MG-related weakness that severely affects the finger flexors and spares finger extensors and intrinsic hand muscles have never been reported. ⋯ The patient had a good recovery with immunotherapy. We conclude that clinicians should be aware that marked weakness of the finger flexors can occur as a result of an MG relapse and may require early aggressive therapy. Case ReportsA Rare Co-Existence of Multiple Cerebral Cavernous Malformations and Pleomorphic Adenoma of the Parotid Gland in Myotonic Dystrophy.
Myotonic dystrophy is an autosomal dominant inherited disorder primarily affecting muscle function. Myotonia, progressive muscle weakness and wasting, and associated systemic involvement, i.e., cataracts, cardiac conduction defects and endocrine abnormalities especially insulin resistance, are the characteristic features. Recent evidence has shown an increased risk of developing benign as well as malignant tumours in such patients. ⋯ Though the association of myotonic dystrophy with salivary gland neoplasms has been sparsely documented in the literature, but the co-existence with multiple cerebral cavernous malformations has not been reported so far. Case ReportsTreatment of a Cyst of the Canal of Nuck with an Inguinal Hernia by Laparoscopic Surgery.
A cyst of the canal of Nuck is a rare cystic structure along the inguinal canal extending to the labium majus, which forms due to non-obliterated processus vaginalis. In female patients presenting with painless swelling in the groin or labium, in addition to strangulated hernia, a cyst of the canal of Nuck should be considered. ⋯ Laparoscopic diagnosis and total extraperitoneal hernioplasty (TEP) offer a useful alternative in patients with a superimposed inguinal hernia on top of the cyst of the canal of Nuck. Case ReportsLaparoscopic Cholecystectomy after Acute Biliary Pancreatitis in a Patient with Situs Inversus Totalis.
Situs inversus totalis (SIT) is a rare disorder defined as symmetrical transposition of thoracic and abdominal organs. Laparoscopic cholecystectomy (LC) is a routine surgical operation and gold standard for symptomatic gallstones. LC can be challenging in SIT due to patient positioning on the operating table and the surgeon's operating position, especially for right-handed surgeons. ⋯ During the operation, trocar sites were mirror images of routine LC and the operation was completed without complications. Diagnosis of acute abdominal pain and abdominal surgery in SIT patients can be demanding.
