Diabetic myonecrosis is an unpublicised problem that can occur in patients having either type 1 or type 2 diabetes mellitus. It usually affects patients who have long-standing, uncontrolled diabetes with associated microvascular complications. We report a case of a 78-year female with diabetic myonecroses admitted to a tertiary care hospital. ⋯ Muscle biopsy can be used for anomalous cases. Although diabetic muscle infarction is a rare pathology, it presents a high risk for diabetes-related morbidity and mortality. Case ReportsA Giant Esophageal Epiphrenic Diverticulum Causing Dysphagia, Fungal Infection and Aspiration Pneumonia in an Elderly Patient.
Esophageal epiphrenic diverticulum is a rare condition. We present a case of a 70-year male with no known comorbidities who presented with dysphagia, cough, fever, and weight loss for 3 months. CT chest with contrast showed an out-pouching arising from the distal dorsal oesophagus, 15.6 cm long with 0.9 cm neck, containing food particles causing compression and consolidation of adjacent lung segments. ⋯ A biopsy of diverticular mucosa showed fungal hyphae and spores and was negative for malignancy. The patient was given antifungals and showed significant improvement of symptoms but considering the huge size of the diverticulum, he was referred to a thoracic surgeon. Case ReportsVaginal Leiomyoma Presenting as Pelvic Organ Prolapse in Pregnancy.
Vaginal leiomyomas are extremely rare in pregnancy. These can present in a variety of ways. They can cause mechanical obstruction leading to maternal and perinatal morbidity. ⋯ She was admitted and a lower segment caesarian section was done followed by transvaginal resection of the mass, and cystoscopy was done. Vaginal leiomyomas require surgical removal and it is usually curative and recommended as the initial treatment of choice, especially when the tumour is large enough to cause a potential obstacle to normal labour. Case ReportsRetropharyngeal Abscess Presenting as Torticollis.
Retropharyngeal abscess (RPA) is a deep neck infection with life-threatening complications such as airway obliteration, necrotizing mediastinitis, and pulmonary empyema, which must be diagnosed early and treated promptly. We herein present a patient who was admitted to the emergency room with limited neck movement, torticollis, difficulty in swallowing, and a feeling that something is stuck in the throat and diagnosed with RPA. Plain lateral radiograph of the neck revealed air levels at the level of C3-C5. ⋯ It is important to recognise and treat RPA, which has a high mortality due to its complications. RPA should be kept in mind in the differential diagnosis of patients with neck pain and torticollis, particularly in adults.
