Case ReportsManagement of Gastric Tumor Perforation with Conservative Treatment during Neoadjuvant Chemotherapy.
Perioperative chemotherapy provides advantage for gastric cancer patients in terms of survival. A 56-year male with a diagnosis of locally advanced gastric carcinoma presented with complaints of acute abdominal pain; and was diagnosed as gastric tumor perforation during neoadjuvant therapy. Gastric perforation may occur during neoadjuvant chemotherapy for gastric cancer. ⋯ We used the minimally invasive treatment approach with nasogastric drainage, intravenous antibiotics and proton pump inhibitors, which was effective in the treatment of perforation. Althrough, conservative treatment approach may be an effective management option in selected patients with gastric cancer perforation. Observational StudyDuct-to-Mucosa Pancreaticojejunostomy with Less Serosal Stitches: A Different Approach to Well-known Problem.
To describe a new approach (duct-to-mucosa pancreaticojejunostomy with less serosal stiches) for postoperative pancreatic fistula (POPF) in pancreaticoduodenectomy. ⋯ Two-layer duct-to-mucosa pancreaticojejunostomy with less serosal stitches technique has acceptable pancreatic fistula rates. This technique could be used by surgeons who are faced with challenges with the duct-to-mucosa anastomosis due to aforementioned causes. Large multi-centre randomised future studies are required to confirm these findings. Maternal Mortality with SARS-COV-2 during its 4th Wave in Pakistan: The Vaccine Paradox and Pregnancy.
The goal of this study was to investigate the maternal death rate among admitted pregnant patients with SARS-COV-2 during its 4th wave in Pakistan. It was a cross-sectional analytical study, carried on pregnant patients admitted due to COVID-19, in Sadiq Abbasi Hospital from 15th August to 15th September, 2021. Thirty-three PCR confirmed and HRCT suggestive patients were included with mean age of 28 ± 4.5 years and mean gestational age of 28.5 ± 6 weeks. ⋯ Regression analysis for maternal mortality predicted by severity and vaccination status was significant with R2=.68, F (1, 31) =66.6, p <.001 CI (-.69, -.42) and R2=.44, F (1, 31) = 24.8, p <.001 CI (-1.14, -.48), respectively. There was substantial mortality in the admitted and non-vaccinated pregnant patients with COVID-19. Key Words: Pregnancy, Vaccination, Severe COVID, Maternal outcome.

Observational StudyMortality in Patients of COVID-19 Infection: Biochemical Markers and its Cut-off Values for Predicting Outcome.
To determine the efficacy and cut-off values of C-reactive protein (CRP), lactate dehydrogenase (LDH), serum ferritin, and D-dimer for predicting mortality of COVID-19 infection. ⋯ Elevated serum CRP, LDH, ferritin and D-dimer are associated with higher mortality in patients of COVID-19 infection. Serum CRP ≥30ng/ml, LDH ≥400 U/L, ferritin ≥200 ng/ml and D-dimer ≥400 ng/ml can predict fatal outcome in COVID-19 patients.
