The aim of this study is to describe the characteristics and outcome of thyroid storm patients presenting at two tertiary care centres of Karachi, i.e. at Dow University of Health Sciences; and Liaquat National Hospital, Karachi, from December 2018 to May 2019. All patients between 18-70 years of age, who were admitted with thyroid storm and fulfilled the Burch- Wartofsky criteria, were inducted. Demographics including age, gender, clinical presentations, systemic symptoms, clinical examination of the thyroid and laboratory findings were reviewed from the file records to identify factors associated with mortality using the available data. ⋯ The mean age was 43 ±1.67 years. Infections were the most common comorbid condition followed by cardiovascular and gastrointestinal diseases. The in-hospital mortality rate was 87.5% (n=07).

Comparative Study Observational StudySurgical Management of Paediatric Empyema: Open Thoracotomy versus Video-assisted Thoracic Surgery.
To compare the effectiveness of open thoracotomy and video assisted thoracic surgery (VATS) for empyema thoracis in paediatric population. ⋯ Early surgical treatment, both open as well as VATS, gives satisfactory results in management of paediatric empyema. VATS is a safe alternative to open thoractomy with good success rate and less incidence of complications.

Case ReportsAngiography and Therapeutic Embolisation of Bleeding Aberrant Iliac Artery Branches.
A 78-year female presented with the complain of per rectal fresh bleeding for 4 days. She was known to have diabetes and hypertension, 3 weeks back. She had an episode of left middle cerebral artery (MCA) stroke. ⋯ Bleeding was initially believed to be coming from sigmoid arteries seen on images of CT scan and colonoscopy. However, arteriography showed that source of bleeding was from middle and inferior rectal arteries that originated from left internal iliac artery. The intervention radiology (IR) team had to put in extensive effort to locate and perform therapeutic embolisation.

Correlation Between the Severity of Coronary Artery Ectasia and Monocyte/Lymphocyte, Platelet/Lymphocyte, and HDL/LDL Ratios.
To evaluate the efficacy of the monocyte/lymphocyte, platelet/lymphocyte, and HDL/LDL ratios as markers of the severity of coronary artery ectasia(CE). ⋯ Although there should be awareness of CE, many clinicians do not pay as much attention to CE as to coronary stenosis. The results of this study showed that the monocyte/lymphocyte ratio and platelet/lymphocyte ratio are consistent with the ectasia severity. This correlation will be useful for the evaluation of follow-up and treatment success in patients with CE.

Case ReportsNeuromyelitis Optica as an Initial Presentation of Systemic Lupus Erythematosus in a Young Male.
Aquaporin-4 antibodies are hallmark of neuromyelitis optica (NMO) and neuromyelitis optica spectrum disorders (NMOSD) that are distinct disease entities. We report a 24-year male with NMO, who presented with Brown-Sequard syndrome. Clinical and laboratory parameters confirmed the diagnosis of systemic lupus erythematosus (SLE). Involvement of sympathetic trunk causing Horner's syndrome and a wide time lapse of 13 years from the occurrence of optic neuritis to myelitis with no relapses in between is a rarity to be seen in NMO associated with SLE.
