To study nociceptive processing in chronic tension-type headache. ⋯ Chronic tension-type headache may represent a disorder of an endogenous antinociceptive system with a lowering of tone and recruitment of descending inhibitory systems.

Topical aspirin in chloroform and the relief of pain due to herpes zoster and postherpetic neuralgia.
To determine pain patterns and relationships in patients with herpes zoster and postherpetic neuralgia before and after topical application of aspirin dissolved in chloroform applied to the painful skin surface. ⋯ Topical aspirin dissolved in chloroform is an effective means of reducing pain due to herpes zoster and postherpetic neuralgia in most patients. The locus of pain origin and analgesia induced by topical aspirin is most likely at cutaneous free-nerve ending pain receptors. The mechanism responsible for the analgesic properties of aspirin is probably not the same as that responsible for its anti-inflammatory properties.

Randomized Controlled Trial Clinical TrialPropranolol and amitriptyline in prophylaxis of migraine. Pharmacokinetic and therapeutic effects.
To determine if the effectiveness of propranolol hydrochloride and amitriptyline hydrochloride are correlated with blood levels and/or with standardized test of pharmacologic effect and to determine which clinical variables are predictors of response to one or the other medication. ⋯ No significant correlations were found between changes in headache score and blood level of drug or change in any of the physiologic measurements. Amitriptyline significantly reduced the severity, frequency, and duration of headache attacks; propranolol reduced the severity of attacks only. Amitriptyline response was correlated with female gender and baseline headaches of shortest duration and of highest frequency. Propranolol response was associated with attacks of greatest duration at baseline and with low pulse rise with exercise at baseline. Nonspecific response was associated with male gender and most frequent headaches by history.

Case ReportsAnatomic-radiologic basis of Lhermitte's sign in multiple sclerosis.
To test the presumption that Lhermitte's sign in multiple sclerosis is the result of a lesion in the cervical spinal cord. ⋯ The findings confirm the presumption that a lesion in the posterior columns of the cervical spinal cord is the cause of Lhermitte's sign in multiple sclerosis.

To summarize 16 years' experience in the diagnosis and treatment of spinal cord decompression sickness in Israel. ⋯ United States Navy air decompression tables appear not to be completely safe for sport divers. Even mild spinal symptoms identified on surfacing should be treated vigorously. High-pressure oxygen-helium therapy seems to be a promising alternative in cases of severe spinal cord decompression sickness.
