Case ReportsUnilateral traumatic hematocele--an unusual presentation of bilateral testicular rupture.
The case of a young male patient with penile laceration and right hematocele following blunt trauma to the genitalia is presented. Both testes were found ruptured at exploration and were repaired. The value of early surgical intervention in scrotal trauma is reiterated and indications for bilateral testicular exploration in cases of unilateral traumatic hematocele are suggested.

Effects of exaggerated lithotomy position on ventilation and hemodynamics during radical perineal prostatectomy.
Ventilation and haemodynamics were studied in nine anaesthetized male patients undergoing perineal prostatectomy in the exaggerated lithotomy position (flexed, head-down position). In the flexed head-down position, as compared to the supine position, there was a significant decrease in arterial oxygen tension (13.1 +/- 2.1 to 11.2 +/- 1.1 kPa, p < 0.001), a significant increase in carbon dioxide tension (4.6 +/- 5.7 to 5.7 +/- 0.2 kPa, p < 0.001) and shunt fraction (7.8 +/- 2.3 to 14.0 +/- 3.0%, p < 0.01). There were also increases in pulmonary wedge pressures (11.3 +/- 2.9 to 17.9 +/- 2.9 mmHg, p < 0.01), mean pulmonary artery pressures (17.6 +/- 3.8 to 25.2 +/- 3.5 mmHg, p < 0.001) and central venous pressures (11.9 +/- 2.1 to 14.7 +/- 2.8, p < 0.05). ⋯ Mean pulmonary artery, pulmonary capillary wedge and central venous pressures also fell significantly. It is concluded that the extreme exaggerated lithotomy used during surgery results in impaired oxygenation and increased cardiac filling pressures but to acceptable levels in healthy anaesthetized patients. All values returned to normal when patient position was normalized after surgery.

Multicenter Study Clinical Trial Controlled Clinical TrialAntiemetic efficacy of prednisolone: a placebo-controlled trial in patients with advanced prostatic cancer treated with estramustine phosphate.
The antiemetic effect of prednisolone on nausea/vomiting was investigated in 67 patients with advanced prostatic cancer and a performance status of < or = 2. The study was a double-blind, placebo-controlled, randomized trial with a parallel group design. The objective was to compare the incidence and severity of nausea/vomiting between the two groups. ⋯ The results indicated that no statistical differences between the two groups of patients were present in AUCs for weeks 1-3 or weeks 1-4. We conclude that it was not possible to demonstrate a significant antiemetic efficacy of prednisolone. However, in all but one case the patients in the prednisolone group could be treated for at least 3 weeks without any major incidents of nausea/vomiting.

Different rates of scrotal involvement in Henoch-Schönlein purpura (HSP) have been reported. We assessed scrotal involvement in 86 children over a 20-year period: 10 patients suffered from scrotal involvement and 9 of them also had arthritis. The possible association between scrotal involvement and arthritis may help the physician in the differential diagnosis of atypical presentations.

To assess the prevalence and characteristics of enuresis nocturna in adults, the treatment they received and the perceived impact, a random sample of 13081 non-institutionalized adults (18-64 years old) were asked to participate in the study in January-March 1996. The response rate was 87%. A personal computer questionnaire included 23 questions on frequency of bedwetting, daytime wetting, treatment and perceived impact. ⋯ It may affect careers, social life and personal relationships. Adults should be more aware that bedwetting is a treatable problem. More information should be given on this issue.
